USMLE Step 1 | Extended 4-Hour Simulation
Online Exam Simulation
Description
Prepare for USMLE Step 1 through an extended four-hour simulation designed to reproduce the pacing, integrated reasoning, and sustained concentration required by the current examination format. The simulation contains 160 multiple-choice questions, organized into eight 30-minute testing blocks. Questions are distributed across the major Step 1 content areas and emphasize application of foundational biomedical sciences to mechanisms of health and disease, diagnosis, therapy, prevention, communication, and interpretation of clinical information. Content may include Human Development; Blood and Lymphoreticular/Immune Systems; Behavioral Health and Nervous Systems/Special Senses; Musculoskeletal, Skin and Subcutaneous Tissue; Cardiovascular System; Respiratory and Renal/Urinary Systems; Gastrointestinal System; Reproductive and Endocrine Systems; Multisystem Processes and Disorders; Biostatistics, Epidemiology and Population Health; and Social Sciences, including Communication and Interpersonal Skills. Questions may integrate pathology, physiology, pharmacology, anatomy, embryology, microbiology, immunology, genetics, biochemistry, behavioral sciences, nutrition, and other foundational disciplines. Clinical vignettes, laboratory findings, tables, graphs, diagnostic images, pathology findings, and other clinically relevant information may be incorporated throughout the simulation. The examination is completed directly through the microscopIA website using a dedicated online testing environment. The platform is multiplatform and may be accessed using a smartphone, tablet, laptop, or desktop computer. During the examination, a live Zoom session remains available with a human microscopIA agent providing real-time technical assistance. The support agent may help with login difficulties, examination access, navigation, platform issues, or similar technical problems. Academic assistance and answers to examination questions are not provided while the examination is in progress. After submission, participants receive feedback for every question, including the correct answer and an educational explanation or review where applicable. This allows the completed simulation to function both as timed examination practice and as a structured study resource for identifying knowledge gaps and reviewing tested concepts. This is an independent educational simulation. It is not an official USMLE examination and microscopIA is not affiliated with or endorsed by USMLE.
